Dear Nipmuc Community member:
My name is Annie McKnight, a member of the Natick Nipmuc Band. I am coordinating information related to health conditions and health care for members of the Nipmuc community. The project is called "The Nipmuc Community Health Survey - 2008". You are being asked to participate in the project conducted through Bridgewater State College with the support of the Natick Nipmuc Indian Council, the Nipmuc Woman's Health Coalition, The Nipmuc Nation, the Historical Nipmucs, and Dudley/Webster Nipmucs.
According to the 2000 U.S. Census for Massachusetts, only 710 people identified themselves as Nipmucs living in Massachusetts. Today's Nipmuc descendents represent the historic Native people of Massachusetts who survived the first practices and policies eventually directed at most all Native people in the United States. This survey will give a voice to these Nipmuc people as we examine their health status while contributing to the literature on the state of Urban American Indian health care for which data is quite scarce.
Your participation in this survey will aid in acquiring information about the health status of the Nipmuc people. The survey will take approximately 20 minutes to complete. The Nipmuc tribal councils and the Elders from the community at large have helped compile a list of people to whom the survey will be directed. Additionally, the Massachusetts Center for Native American Awareness, Nipmucspohke, and the New England Native American Institute are assisting with outreach to all Nipmuc people for inclusion in this "The Nipmuc Community Health Survey - 2008".
All data will be kept STRICTLY CONFIDENTIAL and ANONYMOUS. There will be an ID number located at the end of the survey to determine the response rate. Bridgewater State College requires that you give your agreement to participate in this project. This acknowledgement is also located within the survey on the web site. Your role in this study is voluntary; therefore you are free to stop your involvement at any time.
